FIT Description

Overview

Flexible Integrated Treatment (FIT)
We Empower Youth to Reach their Potential

Thousands of children and youth in our community are suffering from serious mental health conditions that without help, may face significant obstacles to realizing their full potential.

Through Stanford Youth Solutions’ Flexible Integrated Treatment (FIT) program, our community’s at-risk youth of all ages and their families become empowered to lead their lives more fully through our intensive, evidence-based therapy models and linkage to community-based services and supports.

We focus on tangible benchmarks and outcomes throughout our service for each young person and their family. Youth learn new skills to decrease their mental health symptoms and risky behaviors while building on their strengths to improve the overall well-being and behavioral health.

Program Facts

  • Medi-Cal eligible children and youth ages 0 – 18 (and in some circumstances up to age 21) may receive services through FIT for up to 12 months.
  • We always work with families and caregivers, and never work with a child in isolation.
  • We achieve excellent outcomes through our use of evidence-based therapy modalities and best practices including Trauma-Focused Cognitive Behavioral Therapy (TF-CBT), Cue Centered Treatment (CCT), Functional Family Therapy (FFT) and Parent-Child Interaction Therapy (PCIT).
  • Therapeutic services may be provided in the family’s home, in the school setting, in the community setting of the family’s choice, or in our conveniently located counseling center.

Case Management Services

Our team of trained professionals provide comprehensive case management services and supports to youth and their families. Services include:

  • Psychiatric evaluations and medication management.
  • Attendance and advocacy at Individualized Education Plan (IEP) meetings with schools.
  • Attendance and advocacy at Student Study Team (SST) meetings with schools.
  • Attendance and advocacy at Team Decision Making (TDM) meetings with Child Protective Services (CPS).
  • Linkage to community-based resources for transportation, food, clothing, and shelter.
  • Collaboration with schools, doctors, hospitals, social services, child welfare, courts, Alta Regional Center, Alcohol and Drug Services (ADS) and Juvenile Justice.

What Services Look Like

When youth are referred to our FIT program, they first receive a comprehensive assessment to determine their family’s strengths and needs. Staff utilize an array of mental health services and supports that are individualized and based on specific needs. Services will begin with the end of services in mind, always moving towards the goal of wellness.

  • Once a referral is received, a FIT clinician will contact you to set-up an intake meeting within 10 days of you making the referral to Sacramento County Access. Both the youth and caregiver must be present for this meeting.
  • During the first 30 days of service the family and assigned clinician will engage in the assessment process to identify the priority needs and determine the recommended level of service.
  • Youth and families develop a treatment plan, based on the families vision and identified goals, with the support of their clinician.
  • Youth are assigned a FIT Therapist who will coordinate the services identified.
  • Individual and/or family therapy will be provided by a licensed or associate clinician (who is under the supervision of a licensed MFT or LCSW).
  • Children and youth will have access to our art and expressive play therapy center.
  • Child Family Team meetings will be scheduled as needed to help families overcome significant challenges, check-in on their treatment and service plan, and to update their safety plans.
  • Therapy and Child Family Team meetings are held in families’ homes, schools, community settings, or at our conveniently located counseling center.
  • Professional staff are culturally competent, meaning not only are translation services always available, but we understand and respect each family’s unique culture and put it at the forefront of our work together. Families are encouraged to use their natural supports.
  • We create safety plans with families to prevent or utilize in a crisis.
  • We provide 24-hour crisis intervention and on-call family support.
